New Client Forms

PLEASE FOLLOW THESE INSTRUCTIONS CAREFULLY. OTHERWISE, YOUR APPOINTMENT MAY NEED TO BE RESCHEDULED!

  • Before your first appointment, please complete both the Client Intake Form and the Adult or Adolescent Questionnaire (for individuals age 17 and younger) below, as applicable.

  • If we'll be meeting in my office, please remember to BRING ALL COMPLETED PAPERWORK TO YOUR FIRST APPOINTMENT.

  • For teletherapy clients (therapy via video conference or telephone), please email me the completed paperwork at least 24 HOURS before your first appointment. My email address is troygremillion@ptroygremillion.com. Once both forms are completed, please text me the password(s) at 713-492-5420. In your text, please include your appointment date/time and your initials only. 

  • PLEASE NOTE: HIPAA law prohibits me from treating you without a fully-completed Client Intake Form.

  • For teletherapy clients, please be sure to download Zoom, and I will email or text you the Zoom invitation link once the initial appointment is confirmed.

FEES

  • Individual Session - 50 minutes @ $210 or 80 minutes @ $335​

  • Couples Session - 50 minutes @ $235 or *80 minutes @ $375​

  • Family Session - 80 minutes @ $375​

  • Group Psychotherapy Session - 90 minutes @ $90 per member

    *Extended sessions are especially recommended for couples therapy.

NOTE TO COUPLES CLIENTS: EACH of you must complete BOTH the Client Intake Form and Adult Questionnaire.
